Your First Visit

Please arrive approximately 10 minutes early and take time to complete a New Patient form. You can also download this form below, complete it and bring it with you to your visit. 

New Patient Form

• Initial Examination

  Your dentist will start by reviewing your medical and dental history and completing a comprehensive dental exam. During your exam, your dentist will check your teeth, gums, bite and jaw, an oral cancer screening and any X-rays that may be needed. 

• Treatment Plan

  The dentist will create a long-term dental treatment plan for you based on your history and examination. You'll also have time to ask any questions you may have about your dental health and treatment options.

• How can I book an appointment?

  To schedule an appointment at Dr. Sam Lee, please contact our office during our regular business hours.

• How do I cancel or reschedule an appointment?

  To cancel or reschedule an appointment, please call us. Please let us know about any change of plans as soon as you can, so another patient may be able to use the space in our schedule.

• Do you offer appointment reminders?

  In the days before your appointment, you will receive automated phone, text or email appointment reminders from us. If you need to cancel or reschedule an appointment, please call us as soon as possible to let us know.

• Do you provide services in any languages other than English?

  Yes, our office is able to provide dental services in French and Vietnamese.

  If you have a language preference, please advise us when booking your appointment.

• Does your clinic have on-site parking?

  Yes, we have free on-site parking available for all our patients. We are also easily accessible by public transit. 

Direct Billing to Insurance

We're able to bill directly to most insurance plans. For your convenience, we can submit your insurance claim electronically, resulting in faster reimbursement, without you needing to complete paper forms and provide signatures.

We can direct bill your insurance company and our administrative team is happy to assist you in understanding your insurance plan. 

Payment Methods

The full treatment cost must be paid at the time of your appointment.

You will be responsible for any amounts or co-pays not covered by your insurance. These must be paid at your appointment time. 

We accept payment by cash, debit or most major credit cards.
